Cervical Cancer Treatment by Stage သားအိမ်ဝ (သားအိမ်ခေါင်း) ကင်ဆာ အဆင့်အရ ကုသနည်းများ

သားအိမ်ထုတ်ပြီး ကုသထားတဲ့ချစ်မတွေရှိရင် အကြံတောင်းခံပါတယ် ကျွန်မက သားအိမ်ခေါင်းကင်ဆာအဆင့်တစ်လို့အဖြေထွက်ထားပါတယ်ဆရာဝန်က သားအိမ်ထုတ်လိုက်တာ အကောင်းဆုံးလို့ပြောပြီးထုတ်ပစ်ဖို့အကြံပေးပါတယ် သားအိမ်ထုတ်ရင်ဗိုက်ခွဲထုတ်ရတာလားရှင့် ခွွဲထုတ်ဖူးတဲ့သူများရှိရင် ထုတ်လိုက်ခြင်းရဲ့ကောင်းကျိုးဆိုးကျိုးကို သိပါရစေရှင့် ဝေခွဲရဖို့ခက်ခဲလို့ပါ အဆင့်တစ်လည်းဖြစ်သလိုသားအိမ်ကလည်း လုံးဝကောင်းနေသေးပြီး သားသမီးယူလို့လည်း ရတယ်ပြောတယ် သားအိမ်ခေါင်းကင်ဆာအကြိုအဆင့်တစ်ဆိုရင် သားသမီးကရနိုင်တာလားရှင့် သားသမီးက အရေးမကြီးဘူးသားအိမ်သာခွဲထုတ်ပစ်တာအကောင်းဆုံးလို့လည်းပြောကြတော့ စိတ်တွေဝေခွဲဆုံးဖြတ်ဖို့ ခက်ခဲနေပါတယ် ဘဝတူများရှိခဲ့ရင်အကြံပေးအလင်းပြတာလေးရယူချင်ပါတယ်။

IA cervical cancer အဆင့် (၁ အေ)

အဆင့် (၁) ကို IA1 နဲ့ IA2 လို့ ထက်ခွဲထားတယ်။

IA1 ကုသနည်းများ

cold knife conization, a fertility-sparing procedure, for some small cancers တချို့ကို သားသမီးရနိုင်စေဘို့အတွက် အအေးသုံးခွဲစိတ်ဒါးနဲ့ ကုသမယ်။

total hysterectomy with or without bilateral salpingo-oophorectomy ကင်ဆာပြန်ဖြစ်မှာကို ကာကွယ်တဲ့အနေနဲ့ သားအိမ်ထုတ်မယ်။ တချို့ကို မျိုးဥအိမ်နေရာ ချန်ထားမယ်။ တချို့ကို အဲဒါတွေပါ ထုတ်မယ်။

IA2 ကုသနည်းများ

modified radical hysterectomy and removal of lymph nodes သားအိမ် + မျိုးဥအိမ်နေရာ + သက်ဆိုင်ရာ တက်စေ့တွေ ထုတ်မယ်။

radical trachelectomy, a fertility-sparing surgery, and removal of lymph nodes တချို့ကို သားသမီးရနိုင်စေဘို့အတွက် သားအိမ်ကိုယ်ထည်ချန်ထားတယ် + သက်ဆိုင်ရာ တက်စေ့တွေ ထုတ်မယ်။

internal radiation therapy, for patients who cannot have surgery ခွဲစိတ်မရသူတွေကို အတွင်းဓါတ်ကင်နည်းပေးမယ်။

IB and IIA ကုသနည်းများ

radiation therapy given at the same time as chemotherapy ဓါတ်ကင်နည်း + ခီမိုဆေးပေးနည်း

radical hysterectomy and removal of pelvic lymph nodes with or without radiation therapy to the pelvis, plus chemotherapy သားအိမ်နေရာအားလုံးထုတ်မယ် + သက်ဆိုင်ရာ တက်စေ့တွေ ထုတ်မယ်။ တချို့ကိုတော့ ဓါတ်ကင်နည်း + ခီမိုဆေးပေးနည်းပေးမယ်။

radical trachelectomy, a fertility-sparing surgery သားအိမ်နေရာအားလုံးထုတ်မယ် + တချို့ကို ကိုယ်ဝန်ရနိုင်အောင်ချန်မယ်။

radiation therapy alone ဓါတ်ကင်နည်းတမျိုးတည်း

IIB, III, and IVA ကုသနည်းများ

radiation therapy given at the same time as chemotherapy ဓါတ်ကင်နည်း + ခီမိုဆေးပေးနည်း

surgery to remove pelvic lymph nodes followed by radiation therapy with or without chemotherapy သက်ဆိုင်ရာ တက်စေ့တွေ ထုတ်မယ်။ တချို့ကိုတော့ ဓါတ်ကင်နည်း + ခီမိုဆေးပေးမယ်။ တချို့ကို မပေးပါ။

IIB, III, or IVA အများစုကို combination of external and internal radiation therapy အတွင်းဓါတ်ကင်နည်း + အပြင်ဓါတ်ကင်နည်းပေးမယ်။

အသုံးများတဲ့ ခီမိုဆေးတွေက cisplatin or carboplatin ဖြစ်ပြီး၊ ဓါတ်ကင်တာနဲ့ တွဲပေးလေ့ရှိတယ်။

IVB ကုသနည်းများ

Radiation therapy as palliative therapy သွေးထွက်နေတာကို သက်သာစေအောင် ဓါတ်ကင်နည်း။

Chemotherapy and the targeted therapy drug bevacizumab as palliative therapy ခီမိုသာရာပီ + ပစ်မှတ်ထား သာရာပီဆေးပေးမယ်။ အဲဒါလည်း သက်သာရာရစေဘို့ဖြစ်တယ်။

သက်သာရာရစေဘို အသုံးများတဲ့ ခီမိုဆေးတွေက cisplatin, carboplatin, ifosfamide, irinotecan, gemcitabine, paclitaxel, and topotecan တွေဖြစ်တယ်။

Palliative therapy သက်သာရာရစေဘို့ဆိုတာ ကင်တာကိုတော့ နိုင်စေမှာမဟုတ်ပေမဲ့ လူနာ အနေရ ခံစားရ သက်သာစေမယ်။

Treatment of recurrent cervical cancer ပြန်ဖြစ်တဲ့ သားအိမ်ဝကင်ဆာ ကုသနည်းများ

pembrolizumab ခေါ် immunotherapy အင်မြူနိုသာရပီဆေးပေးခြင်း

Radiation therapy and chemotherapy ဓါတ်ကင် + ခီမိုဆေး တပြိုင်နက်တည်းပေးနည်း။

Radiation ဓါတ်ကင် နည်းဆိုတာ external radiation therapy ပြင်ပကင်နည်းတမျိုးတည်း သို့မဟုတ် external and internal radiation therapy အတွင်း + ပြင်ပကင်နည်းတွဲပေးခြင်း။

သုံးလေ့ရှိတဲ့ ခီမိုဆေးတွေကတော့ cisplatin, carboplatin, ifosfamide, irinotecan, gemcitabine, paclitaxel, topotecan, and vinorelbine တွေဖြစ်တယ်။

သက်သာရာရစေနည်း Chemotherapy + the targeted therapy drug bevacizumab တွဲပေးတယ်။ 

(cisplatin, carboplatin, ifosfamide, irinotecan, gemcitabine, paclitaxel, topotecan, and vinorelbine) ဆေးတွေ သုံးတယ်။

အချုပ် - 

- ကင်ဆာမှန်သမျှကို ကင်ဆာအထူးကုဆရာဝန်တွေက လမ်းညွှန်တာကိုသာ လိုက်နာသင့်တယ်။ 

- လူနာတွေ တယောက်နဲ့တယောက် မတူကြပါ။ 

- ဖေ့စ်ဘွတ်ကနေ တယောက်တမျိုးပြောတာတွေကို လိုက်နာရန် မသင့်မှန်ပါ။ 

- အစားအသောက် မစားရဆိုတာ မရှိ။ စားပါဆိုတာလည်း မရှိ။ 

- ကံတရားလည်း အားကိုးပါလေ။
